- $150.00 application fee (non-refundable)
- $200.00 textbook fee (non-refundable)
- PTF Application and fee ($3 for a single and $5 for a couple)
- An original or a certified copy of the child's birth certificate.
- Child's up-to-date immunization record
- Student's social security card
- Copy of most recent report card
- Copies of all standardized test results (TAKS, Stanford, ITBS)
- Recent picture of student
- Completed application form
- Home Language Survey
- Teacher Recommendation Form (5th – 12th grade students only)
- Pastoral Recommendation Form (7th – 12th grade students only)
- Legal documents (if applicable)
- Parent's drivers license and/or ID card
